• Credit application must be completed and signed (download below)
• A copy of your business/state sales tax license
• A copy of a form of advertising: (business card, copy of phone book listing, newspaper ad, etc.)
• Provide a photograph of your business

THIS APPLICATION AND ANY ORDER ACCOMPANYING THIS APPLICATION WILL BE PUT ON HOLD IF ALL REQUESTED INFORMATION IS NOT PROVIDED.
